Background: Reflexology is a form of complementary and alternative medicine; it has gained popularity in the treatment of various conditions, including labour pain. Aim of this study was to evaluate the effect of foot reflexology on pain severity and outcome of labor among primipara. Quasi-experimental design was utilized to fulfill the aim of this study. A Purposive sample of a total 240 primipara was equally divided into two groups (study group comprising 120 primipara and control group comprising 120 primipara). Data were collected through four main tools: structured interviewing questionnaire, modified partograph, Apgar score and numeric rating scale. Results: there was a highly statistically significant difference related to labour pain during the first stage of labour, duration of different stage of labour, and the Apgar score during the 1st and 5th minute of delivery between both study and control groups (P< 0.001**). Conclusion: foot reflexology had positive effect on pain severity of 1st stage of labour, duration of different stage of labour and neonatal outcome among primipara. Recommendation: Reflexology technique is recommended as an alternative non-pharmacological method, which can be applied in maternity hospitals.
