Following the rule of natural tendency of a cartilage to curl against the weakened side, the present study presents a procedure depends on degloving of the auricular cartilage, creation of an antihelical fold by horizontal mattress sutures, then careful excision of perichondrial cover followed by anterior cartilaginous weakening by shaving, then final refining of the degloved cartilaginous skeleton before redraping of the dissected skin over it. 35 ears of 20 patients were operated upon by senior surgeon. Their ages ranged between 4 and 18 years (mean age 8.2 years). The esthetic results were excellent with hundred percent patient satisfaction rate. The postoperative complications either early or late were comparable to the results of many reported studies. The permanently corrected ears fulfilled the criteria for surgical success with high patient-parent-surgeon satisfaction rate.
